Our patient's journey began with a fever, ranging from 100° to 102°F, accompanied by chills and muscle aches. About a week later, a peculiar rash appeared, spreading across her body. She also experienced joint pain and other discomforts. With a medical history including atrial fibrillation, hypertension, and asthma, the puzzle became even more complex.
On the day of her admission, her joint pain intensified, and a high fever of 104°F brought her to the emergency department. Physical examination revealed tenderness in her joints, and laboratory tests showed elevated white blood cells and inflammatory markers. The patient's medical history and initial test results led to a rheumatological evaluation.
And this is the part most people miss... the initial considerations included acute viral illnesses and even Schnitzler syndrome, but the typical markers for these conditions were absent.
